.Excelsior Wellness Family Medicine has a team of board-certified and credentialed medical providers who integrate primary care, medication management and outpatient mental health counseling. This compassionate team works together to create comprehensive care plans that focus on the patient as a whole person.

Brian Nelson, DO, NMM

Brian Nelson, DO, NMM

Dr. Nelson is a native of Spokane and joined Excelsior Family Medicine in 2022. He received a degree in Biochemistry from Western Washington University, and went on to complete a Doctor of Osteopathic Medicine (DO) from Pacific Northwest University’s College of Osteopathic Medicine in Yakima, WA. After completing his residency, Dr. Nelson worked as a provider for over 5 years in Puyallup, before returning home to Spokane. He enjoys being outside gardening or puttering in the yard, camping with his family which includes his wife and two young children.  His favorite pastime activities are cycling, swimming, and backpacking.

Brianna Melvin, DNP, ARNP, FNP-C

Brianna Melvin, DNP, ARNP, FNP-C

Brianna Melvin was born and raised in Michigan, where she attended Grand Valley State University for her undergraduate nursing degree. She moved to Spokane in 2018, where she worked as an RN in adult cardiac and neonatal intensive care.

After discovering a passion for wellness and preventative care, Brianna pursued a graduate degree as a Family Nurse Practitioner at Washington State University. She enjoys working with all ages and values a holistic approach to healthcare.

Brianna enjoys spending time with her husband, family, and golden retriever. Her hobbies include hiking, running, gardening, camping, and traveling.

Daniel Repsold, MD

Daniel Repsold, MD

Dr. Repsold was born in Madrid, Spain, where his parents were missionaries at the time. His family moved to Spokane when he was four years old. He went to Whitworth University for his undergraduate studies and then left for California to attend Medical School at Loma Linda University.

Wanting to return to Spokane, he completed his residency through the Spokane Family Medicine program and is excited to continue practicing medicine in this community. Dr. Repsold was attracted to family medicine because of the long-term relationships physicians establish with patients. As he works with patients to form care plans, Dr. Repsold approaches each patient as an individual to address physical, emotional, mental, relational, and spiritual wellness. Family medicine incorporates all of his interests within the medical field and is the way he can serve others to the fullest.

He and his wife, Stacie, have three daughters, and enjoy reading, movies, food, church, camping, gardening, and spending time with their families.

Annabeth McGuire, DNP, FNP

Annabeth McGuire, DNP, FNP

Annabeth is a graduate of Washington State University, Spokane, having received her Doctorate of Nursing Practice in 2024. She completed her undergraduate degree in Nursing at Gonzaga University. Annabeth worked as a Registered Nurse for six years at Sacred Heart Medical Center working with medical oncology patients. She enjoys art, including acrylics and resin pouring, watching documentaries, and travel. An active exercise enthusiast, Annabeth enjoys cycling, weight lifting, running and stretching. She is also fluent in Spanish.

Kati Kelsey, MA, LMHC

Kati Kelsey, MA, LMHC

Kati joins the EFM team after more than 10 years of working as a community therapist in Spokane. She was born and raised here and attended Western Washington University for her Bachelor of Arts. After a short break from schooling, she attended Gonzaga University for her Master of Arts in Clinical Mental Health Counseling.  Kati has served the Spokane community as a mental health therapist in the school setting, working with a broad age-range of youth and their families. Prior to becoming a therapist, she dedicated time abroad, teaching at an orphanage and providing case management to the children.

Kati is an animal enthusiast and she loves to travel.  Some of her hobbies include: writing, dancing, outdoor sports, camping, hiking and reading.  Kati is a huge football fan and has a soft spot for GU basketball.

Andrew Thurber

Andrew Thurber

Andrew was born and raised in the Spokane Valley, and returned in 2009 when he started graduate school. He received an undergraduate degree in University Studies with emphasis on Humanities, English and Psychology, which helped him realize that he loves the human story.
The work of a counselor aligns with getting to know and understand people’s strengths and struggles. These ideas helped guide him toward becoming a counselor, and he completed a Master of Arts in Community Counseling at Gonzaga University in 2011.

He incorporates a variety of modalities into his counseling work including, EMDR, CBT, DBT, Heart Rate Variability Training, and Art Therapy.

Yajaira Ortiz-Azucar (Yaj) MA, LMHC, MHP

Yajaira Ortiz-Azucar (Yaj) MA, LMHC, MHP

Yaj grew up in California and moved to Spokane to attend Gonzaga University where she completed her undergraduate studies as well as her Master of Arts in Mental Health Counseling. Prior to Excelsior, she worked at Frontier Behavioral Health as both a counselor and as a clinical supervisor. Yaj is part of EFM’s counseling team and also supports the dual-diagnosis program, EXIST.

Outside of work, Yaj enjoys spending time in nature including taking walks, hiking and swimming. Her hobbies include painting, building collages, and creative writing, which make her heart happy. She has 2 cats and a rabbit that she adores, and loves dipping Oreo cookies in frosting to satisfy her sweet-tooth. Yaj is a bi-lingual Spanish provider.

Jeff Glinski, SUDP

Jeff Glinski, SUDP

Jeff works as part of the EFM counseling team, specifically with youth enrolled in the EXIST program. He started with Excelsior in 2008, and through the years served in a variety of roles such as a program manager and campus supervisor. As a substance use disorder provider, Jeff has worked in multiple programs throughout the organization including harm reduction, intensive outpatient and with transitional age youth. Jeff was born and raised in Spokane and earned a degree from Eastern Washington University as well as a Certificate in Addiction Studies.

Outside of work, Jeff enjoys time with family and friends, playing music and building community. He likes going for walks and practicing mediation. He is an explorer and has visited 37 countries with more to come.

Alexia Smith, MS, ARNP, PMHNP

Alexia Smith, MS, ARNP, PMHNP

Alexia joins our psychiatric medication management program through our telepsychiatry partnership. She completed her Master of Science in Nursing at the University of Cincinnati and has worked as a provider for over 10 years. In her prior experience, Alexia held clinical roles in the hospital setting providing psychiatric care and developing mental health service lines. She specializes in child and adolescent psychiatry, and serves all ages for ADHD, depression, anxiety, trauma and PTSD.

Jarred Ford, MA, LMFT Associate

Jarred Ford, MA, LMFT Associate

Jarred began his professional journey in the field of auditing at Ernst & Young LLP in Seattle after earning his undergraduate degree in Accounting from the University of Florida. While working in finance, he explored a deep interest in mental health and began volunteering with the Seattle Crisis Line. This experience inspired a career shift, leading him to pursue a Master of Arts in Marriage and Family Therapy at Whitworth University.

During his graduate training, Jarred completed internships with the Medical Lake School District and the Excelsior WISe team, gaining experience in school-based and intensive outpatient settings, as well as in the Whitworth Marriage and Family Therapy Clinic seeing individuals, couples, and families. He now serves as a therapist at Excelsior Family Medicine, where he brings a thoughtful and relational approach to care.

Outside of work, Jarred enjoys playing pickleball and volleyball, hiking in the Pacific Northwest, and reading literature.

Brittany Haugen, MS, LMHC

Brittany Haugen, MS, LMHC

Brittany grew up in Sedro Woolley, WA, a small town near the Skagit Valley, south of Bellingham. She came to Spokane for college and never left! Brittany attended Eastern Washington University for both her undergraduate and graduate degrees, and received her Master’s of Science in Clinical Psychology. After school, she worked for over 5 years with Frontier Behavioral Health and for the past 10 years worked in Spokane Public Schools focused on middle and high school students. Brittany uses EMDR, CBT and mindfulness in her therapeutic approach.

Brittany is a sports fan and loves Husky football. She enjoys spending time with her family, and her hobbies include being outdoors and reading.
